The PHR Account Holder may make a self-assessment regarding certain symptoms they may be experiencing, concluding that they need an encounter with a provider. This self-assessment should include a specific reason or reasons for the encounter (referred to as the chief complaint(s)) and personal observations or measurements that might be germane to the encounter.
